Each piece has several pages folded as a booklet; inside a single sheet folded to contain them, Qamar Taj Dawlatabadi has written: "The Haykal piece in the auspicious hand-writing of Hazrat Samarah Azal and one in the hand-writing of Hazrat Tahirah."

The notebook's original title is: "Notebook for recording the books currently available at the Hadi Dawlatabadi Library since its founding (5/5/1966), which were transferred from various libraries for sale."
